Filter Patients by Family History in Population Groups

You can now create population groups based on a patient’s family health history. This tool enables practice users to identify patients with specific familial risk factors, facilitating proactive care and personalized health management.  

How it works: 

1) To access Group Creator, navigate to the population management section by selecting patients, population groups, and then select "Create New Group" 

2) Choose "Family History" as a criterion and define Criteria:  

• Select a family member or choose "Any Family Member." 
• Choose health conditions from standardized lists. 
• Use operators to refine your search. 
• Add multiple conditions if needed. 

3) Set Time Frame (optional): Specify time ranges or age of onset criteria where applicable. 

4) Review and Save: 

5) Utilize Your New Group:  Use for targeted outreach, care planning, or risk assessment and easily update or modify as needed.
