Custom Attribute Filtering for Population Groups

You can now filter patients using custom EHR attributes, allowing for more precise and personalized group creation. This tool enables you to leverage unique data fields from your underlying EHR for improved patient management and care coordination. 

How it works: 

1) Access Population Groups:  Patients > Population Groups  

1. Create or Edit a Group:  Select "Create New Group" or choose an existing group to edit. 

2) Add Custom Attribute Filter: Look for the "Custom Attributes" section in the filter options. 

3) Choose from available custom EHR fields currently displayed in Fold. 

4) Set Filter Criteria:  

• For dropdown/boolean fields: Select from available values. 
• For text/number fields: Enter specific values to match or exclude. 
• For date fields: Use the date picker to select specific dates or ranges. 

5) Refine Your Filter:  

• Use matching options like "Exactly Matches," "Contains," or their negatives. 
• Combine multiple custom attributes using AND/OR conditions. 
• Mix custom attributes with other standard filters as needed. 

6) Save and Use:  

• Name and save your group for future use. 
• Apply the group for patient lists, reporting, or care management tasks.
