Configure Automated Wearable Alerts

Practice users can now create population group rules specific to data monitored through wearable devices on Fold. This feature allows users to set up alerts and automate actions based on patient wearable data, such as blood glucose levels, heart rate and step counts, collected from wearable devices.

 

How It Works

 

Creating Pop Groups:


1.    Users can create population groups based on specific conditions related to wearable data on the Fold provider web. Select the Patients Tab > Population Groups > + Create Group


2.    A new rule option named “Home Monitoring” will be available in the dynamic population group creation UI.


3.    Users can select the data point (eg. Heart rate, HRV), frequency, duration, value and other parameters to configure detailed rules. Examples of conditions include “blood glucose above X mg/dL”, or “step count under Y for Z days in a row”.


Note: This option will be available in conjunction with both Athena and Elation as the underlying EHRs.

 

Configuring Automations:

1.    Once a pop group is created, it can be referenced in automations.

2.    A new trigger, "When patients become part of the group," activates the automation.

3.    Automations can include actions such as sending notifications to the care team or creating reminders for patients. Example: If a patient's blood glucose level spikes, an alert can be sent to suggest a quick walking break.


 

Managing Group Membership:

1.    When a patient's data meets the conditions for a pop group, they are automatically added to the group.

2.    If the patient no longer meets the selection criteria in the next execution cycle, they are automatically removed from the group or marked to prevent duplicate actions.
